Make an Appointment

Make an appointment in 3 steps. Step 1: Select a staff member. Step 2: Select an available date. Step 3: Select an available timeslot.

Hello, thank you for making an appointment to apply for your passport!

Please select how many passports will be processed during your appointment. For example, if you and your child are both applying for passports, please select Two Passports (1 hour, 30 minutes). Your appointment may or may not take the full estimated time--we book out plenty of time just in case! If you would like to book an appointment for more than 4 passports, please call 401-828-3750 ext. 4.

Below is a list of what you will need to bring with you to your appointment. Please read this information carefully, as missing materials may require that your appointment be rescheduled.

1. Passport Application. Please bring your application with all fields filled out. You can fill out the application online at Application for U.S. Passport and print one sided (total of 2 pages) or by hand with black ink only. Applications are also available in the Reference Department on the second floor of the library.

2. Photo ID. A valid RI driver's license is acceptable on its own. An out-of-state license or an RI state ID will need to be presented with a secondary ID. Please visit Photo Identification to determine if you need to bring additional ID.

3. Proof of U.S. Citizenship. Common accepted documents are an original U.S. birth certificate with a seal and both parents' names, a previous U.S. passport, or a naturalization certificate. Please visit Citizenship Evidence to ensure you bring the correct documents.

4. Checks or Money Orders. Fees to the U.S. Department of State must be paid in the form of a check or money order. Please visit Fee Calculator to determine your fees ahead of time. The execution fee of $35.00 and any photo fees are paid to the library, and may be paid with cash, check, or money order. If using checks, it is advised to bring them blank and we can assist you in determining what fees are owed.

5. Passport Photo. You may bring your own photo or have us take one for you for $13.00. If you would like to bring your own photo, please visit U.S. Passport Photos for photo guidelines.

For minors under 16, the child(ren) and both parents listed on the child's birth certificate must be present unless the applying parent can provide 1) evidence of sole legal custody, or 2) a notarized form DS-3053 and front and back copy of the photo ID of the non-appearing parent. Parents must bring their valid photo IDs and the child's original birth certificate, even if they have previously been issued a passport. Visit Children Under 16 for more details.

For minors 16-17, parental awareness is required. One parent can accompany the minor(s) with their valid photo ID, but there are other options as well. If the minor has a photo ID of any kind of their own, please bring that also. Visit Applicants Age 16 and 17 for more information.
